Identification forms

Why do I need to complete an Identification Form?
The Anti-Money Laundering and Counter Terrorism-Financing Act 2006 requires the collection and verification of specific information from clients. This varies according to the entity type through which products and services are applied.

When do I need to provide an identification form?
You must supply an Identification Form unless;

  • You were an existing Macquarie client on 12 December 2007 and your account is still open; or
  • You have supplied us with the appropriate Identification Form after 12 December 2007.

Which Identification Form should I use and what do I need to do?
The form you need to complete depends on:

  • Your account type e.g. Individual, Company, Trust etc.
  • If you are applying through a financial adviser or not.

The forms outline the additional documentation that is required to support your application.

To satisfy the identification requirements you need to

  • Have your adviser complete the relevant IFSA/ FPA Identification Form, for the type of account you are opening.
  • Have your financial adviser certify the identification documents specified on the Identification Form.
  • Send us the completed Identification Form and a copy of the supporting identification documents.

What is a certified copy?

A copy of the original documentation which has been signed as a true and correct copy by your financial adviser or another authorised person. The authorised person should print their name and position, and if possible, affix an official stamp.

Individual Identification form for an individual.
Sole Trader Identification form for a sole trader.
Joint Applicants Identification form for joint applicants.
Business Identification form for a business.
Company Identification form for a company.
Corporate Trust or Superannuation fund Identification form for a corporate trust or superannuation fund.
Non Corporate Trust or Superannuation fund Identification form for non corporate trust or superannuation fund.
Deceased Estate Identification form for a deceased estate.
Investing for a child under the age of 18 Identification form for investing for a child under the age of 18.
Partnership Identification form for a partnership.
Incorporated Body or Association Identification form for an incorporated body or association.
Unincorporated Body or Association Identification form for an unincorporated body or association
Registered Co-operative Identification form for a registered co-operative.
Government Bodies Identification form for government bodies.
